Genome tests can identify some early breast cancer patients at low risk of recurrence and without chemotherapy after surgery, researchers in the US and Europe reported in a new issue of the New England Journal of Medicine.This method can be used to predict the survival of cancer patients and to provide a basis for doctors and patients to choose treatment options.
After nearly 6,700 early-stage breast cancer patients in nine European countries underwent surgery, researchers used a method called MammaPrint to detect the marker genes of the removed tumor samples and analyze the risk of cancer recurrence.The researchers also assessed patients' clinical risks using conventional metrics such as tumor size and lymph node metastasis.Patients with high clinical risk usually need chemotherapy after surgery, but chemotherapy has serious toxic side effects.
The researchers screened 1,550 patients with higher clinical risk but lower genomic risk.The patients were randomly divided into two groups, one receiving chemotherapy after surgery and the other giving up chemotherapy after surgery.Five years after surgery, the researchers found that about 95% of patients who did not receive chemotherapy were alive, with a 5-year survival rate of only 1.5% lower than those who received chemotherapy, and no metastasis far from the primary tumor.
This means that early breast cancer patients with higher clinical and lower genomic risks are safe to abandon postoperative chemotherapy, the researchers believe.
Lead author Laura Van Teffeler, a professor at the University of California, San Francisco and inventor of the MammaPrint genome test, said this was the first time that a randomized trial showed that the results could help doctors and patients make wise choices about chemotherapy.
