Recently, Xiao Jun, chief urologist and Tao Tao Tao, deputy chief urologist at the First Affiliated Hospital of China University of Science and Technology (Anhui Provincial Hospital), published a paper in the Asian Journal of Male Sciences.The establishment of this diagnostic model is expected to change the existing diagnostic pattern of invasive biopsy for prostate diseases.
At present, prostate cancer has become the most common malignant tumor in male urogenital system in China.Xiao Jun introduced that due to the lack of specific tumor markers, invasive puncture biopsy is still the current diagnosis of prostate cancer "gold standard."
Recent studies have shown that the molecular genetic background of prostate cancer is highly heterogeneous between European and Chinese populations.But the current prostate biopsy indication in China has been referring to the European and American medical guidelines.
According to the Puncture Biopsy Report of China Prostate Cancer Alliance, following the standards of prostate cancer screening and diagnosis in Europe and the United States, over-treatment of patients with prostate diseases in China is not only unnecessary.
The clinical data and pathological results of 1807 patients undergoing prostate biopsy were analyzed retrospectively. The noninvasive diagnosis model of prostate cancer was established by stepwise logistic regression analysis.
The diagnostic model was divided into two core indexes: prostate specific antigen density test, prostate imaging report and data system score.According to the results of the study, patients with a risk threshold of less than 0.05 had a very low risk of prostate cancer.
Follow-up results showed 99.7% and 99.4% non-diagnosed survival rates of clinically significant prostate cancer at 12 and 24 months post-puncture in patients with negative first biopsy and risk thresholds below 0.05 respectively.When the risk threshold was greater than or equal to 0.60, the probability of prostate cancer was high, and the positive prediction value was 83.5%.
Therefore, it is possible to combine the latest PCT imaging with PSA to achieve radical prostatectomy or local ablation without puncture biopsy.
